This form is to help us understand some important information about you before you come to The Farm. We need to understand your needs, and to make sure that we are providing services fairly and according to need, demonstrating that we are meeting the needs of the whole population.
Once you have filled in the form we will contact you with a start date, this could take up to a week. Please do not come to the farm until then unless otherwise arranged.
